N.P. I got a PM from someone asking me for the registry info. They found an old thread where I has asked a couple of years ago and someone else had provided the information, but the images are now gone. Fortunately, I saved it. Since someone else was interested, I thought it might be useful for others and converted into a PDF file and uploaded.
